Please read over this contract as it explains what I will and will not do for your child, as well as rules and guidelines for payment, and rules for tutoring. By agreeing to work with me, you are agreeing to these terms and conditions.

Please fill out the registration form; out for each individual child and return to me prior to working with your child.

Returning the completed registration form to me will guarantee your child’s spot!
